Note:
a. If one registers for both the “Problem Reporting and Corrective Action” Seminar and the “Root Cause Analysis” Seminar back-to-back (i.e., in the same week), the total registration fee for both seminars is $1495 and the government employee and group discounts also apply.

Registration Cut-off, Discounts and Cancellation / Substitution Policies

Seminar Registration Cut-off Dates:
For each seminar, the registration cut-off date is 22 calendar days prior to the initial presentation date. 
Please register early!

Government Employee Registration Fee Discounts:
A registrant who is a government employee may take a registration fee discount of 10%.  Two or more government employee registrants from the same physical facility each may take a registration fee discount of 15%. 

Group Registration Fee Discounts:
Two registrants from the same non-governmental enterprise, at the same physical facility, each may take a registration fee discount of 10%.  Three or more registrants from the same non-governmental enterprise, at the same physical facility, each may take a registration fee discount of 15%.

Cancellation Policy:
A registration fee will be fully refunded if the cancellation is received twenty two days in advance of the first day of the seminar.  A registration fee will be 50% refunded if the cancellation is received less than twenty two days but more than seven days in advance of the first day of the seminar.  

Substitution Policy:
A substitute may attend the seminar in lieu of the registrant.
